You Want the Muscle… But Do You Know What Steroids Take From You?
If you’ve ever scrolled through social media and thought, “I want that body,” you’re not alone. The problem is, what you’re seeing isn’t always real—and the shortcuts some men are taking to get there are costing them far more than they realize. Today, I want to have an honest conversation about anabolic steroids: what they actually do to your body, why young men are turning to them in silence, and most importantly, what doctors aren’t telling you about the real price you pay.
The Science Behind Anabolic Steroids: How They Work
Let’s start with the biochemistry, because understanding how steroids work is the first step to understanding why they’re so risky.
Anabolic steroids are synthetic derivatives of testosterone, the hormone that naturally exists in your body. When testosterone enters a cell, it binds to androgen receptors and triggers a cascade of effects—everything from muscle growth to hair growth to changes in sexual function. The problem is that testosterone is a 1:1 ratio of anabolic (muscle-building) to androgenic (masculinizing) effects.
Here’s where it gets tricky: scientists have tried to modify the testosterone molecule to increase the anabolic effects while decreasing the androgenic ones. They might create a compound with a 10:1 anabolic-to-androgenic ratio, thinking they’ve solved the problem. But it’s never that simple. Every time you tinker with that molecule, you create downstream effects that are unique to that compound—effects that are often unpredictable and dangerous.
The Cardiac and Metabolic Catastrophe
This is where the conversation gets serious. When men take high doses of anabolic steroids—and I’m talking about doses that can reach 1000 mg per week or higher—their bodies don’t respond the way supplement companies promise.
Here’s what actually happens:
- Lipid changes: Your cholesterol profile tanks. LDL (bad cholesterol) goes up, HDL (good cholesterol) goes down.
- Hypertension: Your blood pressure skyrockets.
- Cardiomyopathy: Your heart muscle weakens and becomes enlarged.
- Coronary artery disease: Plaque begins to form in your arteries, increasing your risk of heart attack.
- Arterial calcification: Your arteries become stiff and brittle.
I’ve had patients come to me thinking they’re invincible because they look strong. But behind the mirror is a cardiovascular system in crisis. A 25-year-old with the heart of a 55-year-old isn’t a victory—it’s a tragedy waiting to happen.
What Steroids Do to Your Liver, Sperm, and Sexual Function
When patients tell me they’re taking multiple oral steroids—like Anavar and Winstrol together—I have to deliver some hard truths. Both of these are 17-alpha alkylated agents, which means they’re designed to survive the first pass through your liver. But that survival mechanism comes at a cost: severe liver stress.
I’ve seen patients develop:
- Cholesterol metabolism problems that accelerate cardiac plaque formation
- Liver damage ranging from mild enzyme elevation to complete hepatic failure
- Suppressed natural testosterone production
- Infertility and sperm count collapse
- Negative sexual side effects that don’t reverse immediately after stopping
Here’s what most men don’t understand: when you take exogenous testosterone (steroids), your body’s natural testosterone production doesn’t just pause—it shuts down. Your pituitary gland stops sending signals to your testes. For some men, that system never fully recovers, even years after stopping.
The Mental Health and Addiction Crisis Nobody Talks About
We don’t talk enough about the psychiatric effects of anabolic steroids. The data is stark: 30% of men who take anabolic steroids develop some form of dependence—either physiological, psychological, or both.
Some of what I see in my practice includes:
- Mood swings and aggression (the infamous “roid rage”)
- Body dysmorphia—the disconnect between how you look and how you perceive yourself
- Psychological dependence that mirrors addiction patterns
- Depression when coming off steroids as your natural hormone production rebounds
And here’s the painful part: many of these young men are self-medicating a pre-existing body image disorder. They’re not getting help from doctors because there’s stigma around anabolic steroids. Instead, they’re getting advice from gym bros and internet forums, compounds from questionable sources, and no medical oversight whatsoever.
Social Media, Body Dysmorphia, and the Unrealistic Standard
Let’s be honest about what’s driving this epidemic: social media. Young men are comparing themselves to filtered, edited, artificially lit highlight reels of people who don’t even look like that in real life. Filters, lighting, strategic angles, and yes, sometimes steroids—all combined into an image that’s fundamentally fake.
But the delta between what they see online and what they see in the mirror feels real. So they think, “How am I going to get there?” And for too many, the answer has become anabolic steroids.
This is body dysmorphia on a massive scale, and it’s being amplified by an algorithm that profits from insecurity.
A Doctor’s Plea: The Harm Reduction Approach
Here’s what I believe: There is enormous stigma around anabolic steroids that has prevented doctors from even learning about them. As a result, patients go off on their own and do their own thing—often with no guidance, no blood work, no accountability.
That has to change. We need doctors who understand these compounds—not to enable them, but to educate, intervene, and harm-reduce. Because the reality is, some men are going to use steroids whether we help them or not. The question is: do we want them doing it in the dark, or do we want an opportunity to guide them toward safer practices?
When a patient comes to me saying they’re using anabolics, I don’t shame them. I educate them. I run their bloodwork. I monitor their cardiac health. I talk about the real risks. And sometimes—just sometimes—I help them realize it’s not worth it. Recently, I made a video about SARMs, and a viewer commented that watching it made him throw away his stash. That’s a win.
But we need more wins. We need more doctors willing to have this conversation openly and honestly.
The Bottom Line
You can’t separate the muscle from the cost. Every anabolic steroid comes with a price tag written in your cardiovascular health, your fertility, your liver function, and your mental health. The guys who look the best at 25 might be facing a heart attack at 40. The confidence boost might come with depression, mood swings, and a dependence you didn’t plan for.
If you’re considering steroids, I’m asking you to really consider the full picture. Talk to a doctor who specializes in men’s health—not a gym bro, not an internet forum, not a coach with a financial interest. Get bloodwork. Understand the risks. And remember: the best body is one that still works at 50.
